VISION AND MISSION
The Medical Records Department shall provide effective and efficient service through its responsible staff, structures and mechanisms.
It is committed to ensure accuracy and completeness of documentation, maintain confidentiality and courtesy in meeting the needs of all its clients.
Requests for Medical Certificate or other Records
• For patients who are still confined in the hospital:
Fill up the Request for Medical Information Form from the Medical Records Department. Records will be given before the patient is discharged.
• For patients who have been discharged:
1. Fill up the Request for Medical Information Form from the Medical Records Department.
2. Secure a written order from the attending physician for the release of the medical certificate or other records and present this to Medical Records staff.
3. A representative of the patient requesting for a medical certificate or other records should present his/her ID together with a written authorization from the patient.
4. If the records needed are any of the following:
Laboratory, X-ray, ECG or Operating Room Records, call up after 2 days to verify if the records are ready. However, if a clinical abstract is requested, call up after 3 days to verify if the records are ready.
5. All records that are requested and released are authenticated by stamping
“CERTIFIED TRUE COPY”
6. Records not taken within 30 days are returned to the storeroom. Another written order from the doctor is required. The process is repeated.
These measures are taken to insure the integrity and protection of the records.
The following are the documents that may be obtained at the Medical Records
Department:
1. Medical Certificate
2. Medico-legal Certificate
3. Certificate of Confinement
4. Patient’s medical records
5. Duplicate/Certified True Copy of Birth Certificate
6. Death Certificate
