STAFF PHARMACIST
Professional Qualifications:
1. B.S. Degree in Pharmacy
2. Duly Licensed by law to practice pharmacy
3. New graduate/or at least 1 year pharmacy practice, orientation with further on-the-job training/ continuing education
Duties and Responsibilities:
1. Fills prescriptions and dispense drugs
2. Compounds preparations not available commercially
3. Labels all drug containers issued to various services
4. Checks the manufacture and expiration dates and labels of drugs
5. Files prescriptions and maintains them for the required number of years as prescribed by law
6. Notifies physicians of any therapeutic incompatibilities, potential drug interactions and duplicate prescriptions
7. Maintains follow up inventory
ACCOUNTING CLERK
Professional Qualifications:
1. Bachelor’s Degree in Accounting
2. Computer literate, proficient in Microsoft Office (Excel, Word, etc.)
Personal Qualifications:
1. Good moral character
2. Honest and trustworthy
3. Hard-working and must be willing to work on holidays and any day of the week
4. Skills in oral and written communication
5. Highly organized
6. Ability to work with minimum supervision
Duties and Responsibilities:
1. provides general accounting support to the finance department.
2. assists in encoding and filing pertinent documents needed in the operations.
3. performs other related tasks as may be assigned from time to time.
MEDICAL X-RAY TECHNOLOGIST
Professional Qualifications:
1. Bachelor’s Degree in Radiologic Technology (BSRT) or
Completion of a 3-year course Associate in Radiologic Technology (ART) and with special training as technologists in Radiology
2. should be a registered X-Ray Technologist (RXT) or
registered Radiologic Technologist (RRT)
3. with at least 3 months to a year training

2. Must be dynamic
3. Highly organized
4. Ability to work with minimum supervision
Duties and Responsibilities:
1. In-charge of performing special and routine radiographic procedures to both in and out patients referred by doctors.
2. Prepares the patient for the examination and contrast material needed for x-ray procedures.
3. Processes exposed x-ray films and checks its quality before forwarding them to the Radiologist for interpretation.
4. Maintains and keeps x-ray files and records properly. Files index cards for control purposes.
5. Responsible for the quality control of film production.
6. Helps in the proper maintenance of the x-ray machines.
7. Prepares chemicals needed for the automatic processor and sees to it that processors are properly maintained.
8. Helps the information clerks in recording, charging of x-ray examinations results, etc. whenever necessary.
9. Assists Senior X-ray Technologist.
10. Informs Senior X-ray tech. of the status of x-ray machines and reports any breakdown Reports other equipment, which need check up and repair.
11. Performs other related duties that may be assigned.
STAFF PHYSICAL THERAPIST
Professional Qualifications:
1. A Bachelor of Science in Physical Therapy and a graduate of a licensed school in Physical Therapy.
2. Passed the Philippine Physical Therapy Board Examination.
3. Experienced in the use of all the physical therapy procedures and techniques.
Personal Qualifications:
1. Mature, responsible and of good moral character.
2. Must have the interest and commitment to treat the handicapped and the disabled.
3. Must be willing to work as a team
4. Youthfulness
Duties and Responsibilities:
1. Interviews patients and his relatives/companions regarding present and past history of illness.
2. Keeps accurate and complete records of patient’s data, PT evaluation, progress notes and treatment programs.
3. Carries out physical therapy procedures as prescribed by the staff physiatrists and accredited physiatrists outside of OLLH.
4. Clears with the physiatrist all direct referrals before carrying out prescriptions.
5. Gives home instructions for follow-up therapy program and self-care activities as prescribed by the physiatrist.
6. Records physical therapy evaluation and makes subsequent progress notes.
7. Accomplishes all clinical data which are directly referred to Physical Therapy.
8. Transports in-patient from room to PMRD and vice-versa.
9. Orients the interns regarding the diagnosis, precaution and management of the patient assigned to the intern.
10. Helps the senior physical therapist in the implementation of the internship training program.
11. Provides clinical supervision to the interns and teaches PT principles, theories and skills to the physical therapy intern assigned to him/her.
12. Edits the initial evaluation, re-evaluation, progress and discharge notes and endorsements of the assigned intern to him/her.
13. Performs other functions as needed.
